What are these documents?

Provincial Transformation Plans are implementation documents. They are complementary to health care strategy documents, which present a long-term vision for the development of the health care system. They take into account the National Transformation Plan, recommended courses of action from the Map of Health Needs and other strategy documents on health care.

What is the structure of the documents?

Provincial transformation plans include:

  1. health needs and challenges of the health system at supra-regional level, as derived from the Map of Health Needs,
  2. actions that are planned to be taken at regional level, including:
    • the planned year(s) of their implementation,
    • responsible parties,
    • estimated cost,
    • expected results,
    • indicators.

Health needs, challenges and actions are shown for each part of the Map of Health Needs, namely:

  • demography,
  • epidemiology,
  • risk factors and preventive care,
  • Primary Health Care,
  • Outpatient Specialist Care,
  • hospital treatment,
  • mental health care and addiction treatment,
  • rehabilitation,
  • long-term care,
  • palliative and hospice care,
  • human resources,
  • Emergency medical services.

In addition, the province governors identified other important areas for regional health care in the plan, such as health resort treatment.
